What We See in the Pool: Foot & Ankle Injuries Explained

Over the past 12 months, we’ve treated a wide range of foot and ankle conditions in our hydrotherapy pool at Aqua Physio.

What’s striking isn’t just the variety it’s how well these injuries respond to early, guided movement in water.

Our latest data shows that largest condition we see in the pool are post Achilles’ tendon ruptures, followed by fractures, sprains, and post-surgical rehabilitation.

But why are these conditions so well suited to hydrotherapy?

Why the Pool Works So Well

Before we dive into each condition, it’s important to understand why hydrotherapy is so effective:

  • Reduced body weight → Less stress on healing tissues
  • Warm water → Improves circulation, reduces stiffness
  • Natural resistance → Helps rebuild strength safely
  • Confidence building → Patients can move earlier without fear
  • Neurological reset → Helps switch muscles back on after injury (especially important in lower limb injuries)

This combination allows patients to start moving sooner, move better, and recover faster.

The Most Common Conditions We Treat

 

Achilles Tendon Rupture (38%)

This is by far the most common condition we see.

An Achilles rupture is a tear of the tendon at the back of your ankle, often occurring during sport. Whether managed surgically or non-operatively, recovery can feel slow and frustrating.

Why the pool helps:

  • Allows early walking with reduced load
  • Helps restore calf muscle activation (often “switched off” after injury)
  • Improves confidence during the early stages

This is especially important because the brain can “protect” the area after injury, limiting muscle function something we work hard to reverse early.

 

Ankle Fractures (32%)

Ankle fractures often involve a period of immobilisation, sometimes surgery, and a gradual return to weight-bearing.

Why the pool helps:

  • Enables walking before full weight-bearing is allowed on land
  • Reduces stiffness and swelling
  • Helps restore normal gait patterns early

Patients often say the pool is the first place they feel like they are “walking normally again.”

 

Ankle Sprains (14%)

Sprains are incredibly common but often underestimated. Poor rehab can lead to long-term instability and recurring injuries.

Why the pool helps:

  • Gentle reintroduction of movement without overload
  • Early balance and control work
  • Safer environment to rebuild confidence

The water allows us to progress rehab faster while still protecting the healing ligaments.

 

Ankle Arthroscopy (8%)

This is a minimally invasive surgery used to treat issues like impingement, cartilage damage, or loose bodies in the joint.

Why the pool helps:

  • Reduces post-operative swelling
  • Restores range of motion
  • Encourages early functional movement

Getting the joint moving early is key to avoiding stiffness and prolonged recovery.

 

Midfoot Surgery (6%)

Midfoot procedures can be complex and often involve restricted weight-bearing, procedures like fusions and Lisfranc Injuries.

Why the pool helps:

  • Allows partial weight-bearing earlier
  • Controlled loading through the foot
  • Gradual return to walking mechanics
  • Prevents deconditioning during recovery

These injuries need careful progression the pool gives us that control and is particularly valuable when patients are otherwise limited on land.

 

CRPS (2%)

Complex Regional Pain Syndrome (CRPS) is a chronic pain condition that can develop after injury or surgery.

Why the pool helps:

  • Warm water helps reduce pain sensitivity
  • Gentle movement improves circulation
  • Provides a safe, less threatening environment for movement

For many patients, the pool is the first-place movement feels possible again.
