225 Days Later: How Hydrotherapy Helped Me Return to Cycling After a Femur Fracture
It’s been 225 days since I was knocked off my bike, suffering a transverse/comminuted fracture of my right distal femur, which, in simpler terms, means the big bone in my leg snapped in half just above my knee, with a few extra fragments floating around.
The Crash That Changed Everything
The injury was severe, but modern surgical techniques (inserting an intramedullary nail) meant that the bone could knit itself back together. That said, I came out of surgery with a big question mark over my future as a cyclist. Would I ever ride again? Would I ever regain my strength, endurance, and confidence?
I was encouraged by my land-based physio to start hydrotherapy, and looking back, it was one of the best decisions I could have made.
Hydrotherapy: The Game-Changer for Fracture Recovery
1. Early Rehab: Moving Without Pain & Gravity
The first few months after surgery were the hardest. My leg was weak, swollen, and stiff. Every movement was a struggle. Walking was impossible, let alone getting on a bike.
Enter hydrotherapy. Being in the pool took gravity out of the equation and let me move without pain, something that would have been unbearable on land.
- Maintained range of motion when stiffness was setting in.
- Reduced swelling thanks to hydrostatic pressure.
- Gently activated muscles without loading the healing bone.
2. Mid-Rehab: Strengthening Without Risk
As my fracture healed, I needed to rebuild my strength and endurance, but pushing too hard on land risked reinjury and setbacks.
In the pool, I could:
- Increase resistance gradually without stressing my bone.
- Strengthen my quads, glutes & hamstrings safely.
- Use deep-water running & aqua cycling to maintain fitness.
While I couldn’t ride outside, I could replicate the pedalling motion in the water, keeping my cycling muscles engaged without impact.
3. Late Rehab: Sport-Specific Conditioning & Confidence
By the time I was cleared to get back on the bike, I was already ahead of schedule thanks to hydrotherapy. My transition to land-based training was smoother, faster, and less painful than I ever expected.
The biggest benefits at this stage?
- Cardiovascular fitness stayed high- I wasn’t starting from scratch.
- Explosive power & endurance came back quicker than expected.
Confidence soared- I didn’t feel like a fragile post-op patient, I felt like an athlete again.
225 Days Later: Back to Peak Performance
Now, just over seven months post-fracture, I’m back on my bike, stronger than ever.
- Cycling FTP? Back to (or slightly above) pre-crash levels.
- 5K times? Getting faster every week.
- Skiing? Nine days on the slopes already!
Hydrotherapy wasn’t just rehab, it was a key part of my comeback. It allowed me to keep moving, stay strong, and rebuild without pain, setbacks, or fear.
