Case Study: How Early Hydrotherapy Transformed an Achilles Rupture Recovery

The road to recovery can seem daunting, especially with a long period of immobilisation in a boot. But what if there was a way to keep moving, stay strong, and make a smoother return to sport?

This is exactly what happened for one of our patients at AquaPhysio. His hydrotherapy journey changed his entire rehab experience, and today, he’s back on his feet, feeling stronger than ever.

The Injury: A Sudden Setback

This 49-year-old investment banker had been training hard, balancing work in the city with a fitness routine that included spinning and running. But after one interval session, he stood up and felt an intense pain in his Achilles.

A trip to A&E confirmed the worst, a ruptured Achilles tendon. Since his ultrasound showed the tendon ends were still together, he opted for a conservative (non-surgical) approach, spending:

  • 4 weeks in a plaster cast in plantar flexion
  • 4 weeks in a boot with 5 wedges, removing one per week

While the tendon was healing, his calf muscles weakened, and he developed compensatory movement patterns that altered his walking.

Initial Assessment: Struggling to Walk

  • 1.5 cm Calf Muscle Loss: Significant muscle atrophy in the injured leg.
  • Thickened, Red Achilles Tendon: Stiff and swollen from immobilisation.
  • Altered Gait Mechanics: Struggling to walk properly, flicking his leg outward, externally rotating his hip, and hitching his leg due to limited ankle movement.

He told us his goal was simple, to return to running. But first, we needed to restore strength, control, and confidence in movement.

Hydrotherapy: The Game-Changer in His Recovery

By getting him into the hydrotherapy pool, we fast-tracked his recovery, helping him safely relearn movements without pain or fear. The properties of water allowed him to:

  • Walk normally without compensations – buoyancy reduced the load on his healing tendon
  • Start strengthening immediately – calf raises were possible from the first session
  • Improve ankle mobility – warm water and movement prevented stiffness

Progress Timeline: From Walking to Jumping in Just 5 Sessions

  • Session 1: Walking with proper mechanics, double-leg calf raises in the water
  • Session 3: Running against the current on the incline pad
  • Session 5: Jumping and landing on underwater plyometric pads

This progression was unmatched compared to land-based rehab alone, where many patients struggle to regain strength and movement after weeks in a boot.

After 8 hydrotherapy sessions, this patient walked normally, his calf strength had returned, and he was back to cycling. His Achilles rupture was almost undetectable in his movement.
